Here is our 3-Step Referral Process
Step 1 – Submit the Referral
Complete the short online referral form with basic details about the participant and the supports required.
Step 2 – We Review and Contact You
Our team will review the information and contact you within 1–2 business days to discuss the referral and confirm support needs.
Step 3 – Support Begins
Once supports are confirmed, we work with the participant, family and support team to put the right supports in place. Our goal is to make accessing support simple, respectful and person-centred.
